What we collect

If you submit a contact or employment form on this website, we receive the fields you enter, plus basic technical data the form processor may attach (such as IP address). Please do not send Medicaid numbers, full medical histories, or other protected health information through those website forms. Hospital and facility referrals use a separate Google Workspace form on Refer a Patient, not the contact form.

How we use it

Contact and job form submissions go to our intake team so staff can follow up. Questions about this policy? Call us at (631) 608-8523. We do not sell personal information.

Analytics

This site is hosted on Cloudflare Pages. Cloudflare may load a Web Analytics beacon to count visits. We load Google Tag Manager and Google Analytics 4 to count visits and see which pages people use. We load WhatConverts for call tracking, and we may use it on contact and employment forms. Refer a Patient stays off WhatConverts so that form stays on Google Workspace under a BAA. We do not load advertising pixels. Do not put protected health information on this website. Cloudflare is not used as a clinical record.

Processors

Contact, employment, and HHA training forms use a Google Workspace web app (same pattern as Refer a Patient). Facility referrals use BSHC's Google Workspace referral form. Hosting is Cloudflare Pages.
