Medicaid and MLTC home care in Nassau
For many Nassau families, Medicaid is how home care gets paid for. The usual order of steps is below. Medicaid itself decides who qualifies; we are a home care agency, not a Medicaid planner, but we can walk you through it.
- Check Medicaid first. Eligibility depends on income and assets. If your parent or spouse does not have Medicaid yet, call us and we will explain where to start.
- The state assessment (NYIAP). Once Medicaid is in place, the New York Independent Assessor Program (NYIAP) arranges an assessment of how much help is needed at home. NYIAP can be reached at 1-855-222-8350.
- Pick an MLTC plan. Adults who need ongoing home care usually enroll in a Managed Long Term Care (MLTC) plan, and the plan approves the hours.

Medicaid Managed Long Term Care (MLTC)

Private pay
Paying privately skips the Medicaid application and is often the fastest way to begin. Some Nassau families use it on its own. Others use it to add hours beyond what a plan approves, such as weekend or evening coverage. Call for current rates.
NHTD and TBI waivers
The Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) waivers are Medicaid programs that help eligible people stay at home rather than in a facility. When authorized, we provide NHTD and TBI waiver services in Nassau. Through this agency, those waivers are offered in Nassau and Suffolk only, not in Queens.
Long-term care insurance
We work with long-term care insurance. Policies differ in what they cover for care at home, so have the policy details ready when you call.
Other coverage
Workers' compensation can pay for home care after a job injury once the carrier authorizes it. We do not bill Medicare. We are a licensed home care agency, not a Medicare-certified home health agency.
