What is an NHTD waiver?
The Nursing Home Transition and Diversion (NHTD) waiver is a Medicaid program designed to help eligible people with disabilities and chronic illnesses move from nursing homes or other institutional settings back into the community, or stay in the community instead of entering a facility. The waiver provides access to personalized services and supports so people can live more independently.
What is a TBI waiver?
A Traumatic Brain Injury (TBI) waiver is a state-administered Medicaid program for people living with the effects of a brain injury. It offers support beyond many traditional insurance plans, with the goal of helping people live as independently as possible in the community.
What services can the NHTD / TBI waiver include?
New York's waiver programs can include a range of supports. Examples include:
- Service coordination, to help with enrollment and oversee the service plan.
- Community Integration Counseling (CIC), for emotional support as people adjust to community living.
- Home and Community Support Services (HCSS), including supervision and personal care with ADLs and IADLs when needed.
- Independent Living Skills Training (ILST), to assess and build daily living skills.
- Positive Behavioral Intervention and Support Services (PBIS).
- Structured day programs.
- Transportation.

Eligibility
Primary requirements typically include active Medicaid coverage; age 18 to 64 with a verified physical disability, or age 65 or older, when applying to NHTD; a physical disability or traumatic brain injury that results in a nursing-home level of care; and the ability to live in the community. Exact rules are set by New York State and can change. We will help you check current criteria.
Enrollment timing
After eligibility is confirmed, enrollment often takes about three to four months when the person already has housing, informal support, and a designated service coordinator. Complex cases can take longer.
