Extra help coordinated with the care plan: live-in aides and partner services.
Beyond our core nursing and personal care programs, Bryan Skilled Home Care coordinates additional support that fits the authorized plan of care. These offerings are not a substitute for a second clinical program. They are extra help, scheduled and supervised with the same care-plan discipline as the rest of our work.
Live-in aides stay in the home so needs can be addressed around the clock as authorized. Duties can include errands, light housekeeping, laundry, meal planning, and help with activities of daily living such as feeding and dressing, always according to the care plan.
We collaborate with Remote Focus on monitoring technology that can support general health and wellness, chronic-condition monitoring (such as COPD and diabetes), and telehealth. Remote Focus can be reached at (929) 630-4100.
We partner with NY Best Medical to help home care patients stay current with New York State Medicaid compliance steps, including annual home care authorizations.
